FRCEM Exam — Complete Guide 2026
Everything you need to know about the Fellowship of the Royal College of Emergency Medicine —
structure, fees, eligibility, pass rates, venues, attempts, and proven strategies to pass both components.
What is FRCEM?
The Fellowship of the Royal College of Emergency Medicine (FRCEM) is the highest postgraduate qualification
awarded by the Royal College of Emergency Medicine (RCEM) in the UK. It is the definitive credential for doctors
pursuing consultant-level practice in Emergency Medicine in the United Kingdom and Ireland.
FRCEM builds on the Membership qualification (MRCEM) and assesses readiness for independent consultant-level
decision-making — encompassing advanced clinical management, leadership, governance, research literacy,
and complex resuscitation. The Fellowship consists of two independent components that can be taken in either order:
📝
FRCEM SBA
Single Best Answer theory exam — 180 questions across two 2-hour papers, mapped to the full RCEM 2021 Emergency Medicine Curriculum. Delivered via Pearson VUE worldwide.
🏥
FRCEM OSCE
Objective Structured Clinical Examination — 16 eight-minute stations assessing consultant-level clinical and communication skills. Delivered in-person in London only.
🗂 History note: Prior to August 2021, the Fellowship also included the FRCEM Critical Appraisal and FRCEM QIP components. Both have since been withdrawn as standalone examinations. Their competencies are now embedded within the SBA and OSCE.
Eligibility Requirements
Candidates must meet all requirements at the point of application, not at the date of the exam. Both FRCEM components share the same core criteria.
🎓
Medical QualificationHold a medical qualification approved by the General Medical Council (GMC) for registration purposes.
🏆
MRCEM (or Intermediate Certificate)Hold full MRCEM (awarded after August 2012) or the FRCEM Intermediate Certificate, obtained within the preceding 7 years.
🏥
Clinical Experience — UK/Irish TraineesCompleted at least 12 months (FTE) of Emergency Medicine training at ST4/ST5 level or equivalent in an approved UK or Irish training programme.
🌍
Clinical Experience — Non-Trainees (IMGs)Completed 6 years of post-qualification training — at least 4 years in Emergency Medicine and at least 1 year at a level above ST3/SHO.
🗣
English Language ProficiencyIELTS Level 7.0 (or equivalent) is the expected standard for all FRCEM examinations.
🔄
OrderCandidates may sit the FRCEM SBA and FRCEM OSCE in any order — there is no mandatory sequence between the two components.
⚠️ Currency rule: After completing MRCEM, candidates have 7 years to take and pass their first FRCEM component. Once they have passed one FRCEM component, they have a further 7 years to complete the remaining component.

OSCE Oversubscription Priority
When demand exceeds places, seats are allocated in this order:
- Candidates in approved ST6 UK training posts
- Candidates in Year 7 of an approved Irish training programme
- Candidates with an NHS employment contract
- Candidates not allocated a place at the most recent sitting
- All other applicants — first come, first served
⚠️ Do not miss the OSCE window. The 7-day window closes automatically at 16:00 UK time. Missing it means waiting for the next sitting — potentially 6 months away. Set a calendar reminder for 10:00 UK time on the opening date as soon as dates are published on the RCEM Exam Calendar.
Exam Fees
Fees depend on your RCEM membership status — members pay significantly less. A tax surcharge may be added
based on your country of residence (e.g. India has an 18% OIDAR government tax on all online RCEM services).
Always check the official RCEM Exam Calendar & Fees page for current amounts.

FRCEM OSCE — London Only
🇬🇧
London, United Kingdom
The FRCEM OSCE is only delivered in London. All candidates — including international doctors — must travel to the UK. This is a key logistical and financial consideration for IMGs.
🛂 Visa note: If your UK visa is declined for a scheduled sitting, contact exams@rcem.ac.uk immediately with proof of refusal to request a refund. Plan visa applications well in advance.
Pass Rates
FRCEM pass rates are significantly lower than MRCEM. Both components are challenging at consultant level.
FRCEM SBA
~47%
Overall pass rate (2023 RCEM data)
The pass mark is determined by the modified Angoff method and varies each sitting. For example, the pass mark
for October 2024 was 108/180. One Standard Error of Measurement is added to the cut score.
FRCEM OSCE
RCEM does not publish a single headline FRCEM OSCE pass rate. Pass rates vary considerably by candidate group —
UK trainees in approved ST6 posts consistently outperform other groups. International candidates face the greatest
challenge, primarily due to differences in communication style, consultant-level decision-making expectations,
and UK-specific protocols.
📊 Key insight: The FRCEM OSCE assesses consultant-level practice — not simply clinical knowledge. Examiners expect leadership, governance awareness, and decisive resuscitation capability. Many candidates who pass MRCEM OSCE underestimate the step up required for FRCEM.
Attempts Allowed & Currency Rules
🔢
Maximum Attempts
4 attempts per component. FRCEM SBA and FRCEM OSCE are each limited to 4. Attempts taken after August 2016 count towards this limit.
📅
7-Year Currency Windows
After completing MRCEM: 7 years to take and pass your first FRCEM component.
After passing one FRCEM component: 7 years to complete the second.
⚖️
Additional Attempts
In exceptional circumstances, additional attempts may be granted at the Dean's discretion. Candidates diagnosed late with neurodiverse conditions can apply to expunge prior unsuccessful attempts.
♻️
Failing to Complete
If you fail to pass one component within 7 years of the other, you may be required to resit the earlier component — subject to remaining attempts.
Validity & Global Recognition
FRCEM is one of the most internationally respected Emergency Medicine qualifications in the world, awarded by a UK Royal College. Once awarded, FRCEM does not expire as a qualification.
🇬🇧
United Kingdom
The standard fellowship for EM consultants. Required for GMC specialist registration via CCT/CESR. Essential for NHS consultant posts.
🇮🇪
Republic of Ireland
Fully recognised by the Irish Medical Council. FRCEM doctors can work as consultants in Irish Emergency Departments.
🌏
Middle East & Asia
Widely recognised by health authorities across the Middle East, Southeast Asia, and South Asia. Particularly valued in Saudi Arabia, UAE, Qatar, Malaysia, and India.
🌍
Africa & Other Regions
Recognised in several African countries and increasingly accepted by health ministries globally as evidence of consultant-level EM competence.
Pros & Cons of FRCEM
✅ Advantages
- Gold-standard UK Royal College qualification — globally recognised and prestigious
- Required for UK NHS consultant posts via CCT/CESR
- Flexible order — SBA and OSCE can be taken in any sequence
- SBA worldwide via Pearson VUE — no UK travel needed for the theory component
- No negative marking in the SBA
- Enhances career prospects across UK, Ireland, Middle East, Asia, and Africa
- IMGs can qualify without being in a formal UK training programme
- Reduced exam burden — Critical Appraisal and QIP components withdrawn since 2021
❌ Disadvantages
- OSCE in London only — international candidates must travel, with significant cost and visa requirements
- Only 4 attempts per component — fewer than MRCEM (6 attempts)
- 7-year currency windows create time pressure for non-trainees
- Low SBA pass rate (~47%) — one of the most challenging written EM exams globally
- Consultant-level expectation — the step up from MRCEM is significant and frequently underestimated
- Limited OSCE places with oversubscription risk for non-trainees
- Cost — exam fees plus UK travel and accommodation make it expensive for international candidates

How to Prepare for FRCEM
FRCEM requires a step-change from MRCEM preparation. The exam assesses consultant-level judgement, governance, and clinical autonomy — not just core EM knowledge.
01
Map everything to the RCEM 2021 SLOs
Download the RCEM EM 2021 Curriculum before opening any textbook. Every SBA question and OSCE station maps to a specific SLO. Understanding the blueprint lets you allocate revision time strategically — particularly to the high-yield SLO3 (resuscitation, ~40 questions) and SLO5 (paediatrics, ~30 questions).
02
Do not neglect SLO10, SLO11, and SLO12
Research & statistics, quality improvement, and education together account for approximately 20 questions. Learn basic statistics (NNT, likelihood ratios, sensitivity/specificity), QI methodology (PDSA cycles), and teaching frameworks (Pendleton's rules, SNAPPS).
03
Know UK-specific guidelines — not just global ones
The FRCEM is mapped to NICE, RCEM, BTS, JBDS, and RCOG guidelines. Many IMGs know the clinical management but apply non-UK protocols and lose marks. Revise NICE guidance for ACS, sepsis (Sepsis 6), stroke, PE (PERC/Wells/CTPA pathway), DKA, and paediatric fever (NICE NG51).
04
Prepare for the OSCE as a consultant — not a registrar
The FRCEM OSCE is set at consultant level. You will be expected to lead a team, make governance decisions, delegate appropriately, deal with complex ethical scenarios, and demonstrate patient safety leadership. MRCEM OSCE preparation alone is insufficient.
05
Master resuscitation — it is a mandatory pass requirement
You must pass at least one resuscitation station to pass overall. Practise ALS, paediatric resus (APLS), major haemorrhage protocol, and RSI until they are automatic. These stations reward preparation and penalise hesitation heavily.
06
Practise SBA timing rigorously
90 questions in 2 hours = ~80 seconds per question. Many candidates run out of time. Practice timed mock papers from the start — develop a strategy for flagging and returning to difficult questions efficiently.
07
Book your OSCE early and plan your travel to London
The FRCEM OSCE is London-only with limited places. International candidates must plan visa applications, flights, and accommodation well in advance. Apply at exactly 10:00 UK time on the opening date — non-trainee candidates are prioritised last under the oversubscription policy.
08
Become an RCEM member before applying
Membership saves significantly on all FRCEM fees. Apply at rcem.ac.uk at least 24 hours before the application window opens — payment must be processed and reflected in your account before you can access member rates.
09
Respect the 4-attempt and 7-year limits — plan strategically
With only 4 attempts per component and 7-year currency windows, strategic planning is essential. Do not defer attempts indefinitely. Each failed attempt consumes one of your four — running out of attempts within the currency window is a career-limiting outcome.
10
Review your RCEM feedback letters after every attempt
RCEM provides detailed feedback letters after all FRCEM exams. Download your feedback from your RCEM account and discuss it with a supervisor or mentor. Domain-specific weakness data is invaluable for focused remediation before your next attempt.
